Dental Patient Information
Welcome to White Pine Family Dentistry!
When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office.
Your initial exam will last approximately one hour. Oral hygiene instructions will be provided along with suggestions to help you care for your teeth. In most cases, we will also clean your teeth on this visit and provide an evaluation that will outline your existing dental problems and proposed treatment.
What to Expect
On your first visit, you can expect:
- An introduction to our office and dental team
- A thorough examination and review of your oral health, including necessary X-rays and existing restorative charting
- A thorough examination and review of your whole body health history, including any medications and herbal supplements you are taking
- A careful evaluation of your current dental status including an oral cancer check and caries (decay) exam
- A discussion of the most satisfactory treatment plan to meet your oral health goals
- A warm and friendly dental team focused on your needs
Following a careful and thorough diagnosis, we will discuss a treatment plan that is suitable for you. This treatment will provide optimum dental health within well-planned appointments.
Appointments
Your scheduled appointment time has been reserved specifically for you. We request a 48-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation.
Financial
It is our mission to provide you with affordable, high-quality treatment that fits your budget! We offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, and Visa, MasterCard, American Express and Discover credit cards.
Insurance
Our office understands the value of insurance benefits to our patients. As a courtesy to our patients, we will accept assignment of your primary benefits towards the payment for your dental treatment. Dental plans do vary greatly, and we encourage our patients to know the details of their plan to eliminate disappointment with payments and reimbursement. Due to the complexities of insurance contracts, we can only estimate your coverage in good faith but cannot guarantee coverage at the time of service. Your estimated patient portion must be paid at the time of service. Insurance claims that are not reimbursed to us within 30 days will become the responsibility of the patient. Upon request, we can provide a pre-authorization for estimated insurance benefits in advance of treatment to enable patients to prepare their finances accordingly.
